What is a Certified Registered Nurse
Practitioner (C.R.N.P.)?
The C.R.N.P. may practice medicine under
physician supervision. Most likely, the nurse practitioner is whom a patient will first
see during a medical visit. The nurse practitioner assesses a patient's health status by
attaining health histories, performing physical exams and ordering and interpreting
diagnostic tests. The nurse practitioner consults with a physician and establishes a
health care plan.

Hamilton received her Post-Masters Certificate as Family Nurse Practitioner from Penn
State University. She obtained a Master of Science in Nursing (M.S.N.) from College
Misericordia and a Bachelor of Science in Nursing (B.S.N) from West Virginia University.
Ms. Hamilton earned an Associate Degree in Nursing from Hagerstown Junior College after
receiving a Bachelor of Arts from Shippensburg University.

What is a Physician Assistant?
A physician assistant
(PA) is a health care professional licensed to practice medicine with
physician supervision and is trained to do many tasks that physicians
usually do. As part of their responsibilities, PAs conduct physical
exams, diagnose and treat illnesses, order and interpret tests, counsel
on preventive health care, and in Pennsylvania can write prescriptions.
PAs must complete an accredited, formal education program and pass a
certifying exam. Those PAs who pass the exam may use the credential
“PA-C.”

Ms. Pellegrino completed the Physician Assistant Program and earned an
Associate Degree at Hahnemann University in Philadelphia, PA. Prior to

General Patient Information for Keystone Family Medicine
Office Hours
- Monday, Tuesday, Friday 8:00 am 6:00 pm, Wednesday,
Thursday 8:00 am 8:00 pm,
Saturday 8:00 am noon.
Emergencies When the Office Is
Closed - If you have a serious emergency, immediately call 911 or go to the
hospital emergency room. When the office is closed, if you have a serious medical
problem that is not an emergency but cant wait until we reopen, call 263-4313 and
our answering service will have a provider call you.
Appointments
- Call 263-4313 to make appointments. Whenever
possible we will make the appointment with your chosen provider. However, there may
be times when he or she is not available at the time you would like your
appointment. In that case, you will be scheduled with a member of your
providers team.
Please arrive 15 minutes before your scheduled appointment to allow time for check-in,
where we make sure our information about you is correct. We will do our best to be
on time and appreciate your understanding when emergencies happen that put us behind
schedule.
Cancellation of Appointments
- If you cant keep your appointment, please call 263-4313 and then press
option 1 to let us know. We ask that you cancel at least 24 hours before your
appointment, if possible, so that we can give another patient that time.
Prescription Refills
- Please call 263-4313 and then press option 7 a few days before your prescription
runs out. Because other patients are also asking for refills, we need at least 2
business days to get your prescription ready. Please note that prescriptions cannot
be refilled during the weekend.
Lab and Test Results
- At your appointment, your nurse or provider will tell you how to get your lab
results.
Medical Records
The information in your record is confidential and will not be given out without your
written permission. If you want your records given out, please come to the office or
mail your request telling us where the records should be sent. There may be a charge
for this service.
Return of Telephone Calls
- Because our providers have a full-schedule of patients to see each day, they may
not be able to return your call until the end of the day or a nurse may return your call
on their behalf.
Payment for Services
- Services are available to
everyone. We participate with most insurances and accept Medical
Assistance and Medicare. We have a sliding fee scale for those who
qualify, and we will work with patients to set up a payment plan if
necessary. Your payment is due at the time of service unless other
arrangements have been made.
Billing Questions -
Call our Billing Office at 709-1222 Monday through Friday from 8:00 am to 5:00 pm if
you have a question about your bill.
Complaints, Suggestions, or
Comments - We welcome your concerns, suggestions, and comments so we can continue
to make our services better. Please talk to any of our staff if you have a concern or
comment. If your concerns remain unanswered, you have the right to
notify the Joint Commission on Accreditation of Healthcare Organizations
at www.jointcommission.org or by calling

Turn left on Norland Avenue (Just as you reach the Wilson College campus, Rt. 11 makes a
sharp left turn. After you make that turn, you will be on Norland Avenue. Move to the
center lane.) Follow Norland Avenue 0.6 miles. Keystone
Family Medicine is in the building
on the right, named Keystone Health. Turn right into the parking lot.
From Rt. 11
going North:
Follow Rt. 11 North to Queen Street. Turn right on East Queen Street (Rt. 30 East/Lincoln
Way East). Follow East Queen Street for 0.3 miles. Turn left on Fifth Avenue. Follow Fifth
Avenue for 1.0 mile. Go straight through the intersection of Fifth and Norland Avenues.
Keystone Family Medicine is in the building on the right, named Keystone Health. Turn
right into the parking lot.
From I-81,
Exit 17 (Walker Road): From I-81 take exit 17. At the end of the ramp, exiting from
the north turn right, exiting from the south turn left, onto Walker
Road. Go to the light and turn right onto Norland Avenue.
Drive 0.8 mile to the light at Norland and Fifth Avenues.
Turn right onto Fifth Avenue. Keystone Family Medicine is in the building on the right, named Keystone Health. Turn
right into the parking lot.
